Becoming A Patient

Because pain management is a specialized medicine, it's best to check with your insurance company to find out their referral policy and covered services, as well as to to find out if we're one of their In-Network providers. You may also find the answer on our Insurance page.

To set up your first visit, simply give us a call. Be sure to have the following pertinent information ready:

  • First & Last Name (be sure to verify spelling, i.e. Sheryl or Cheryl, Matthew or Mathew)
  • Date of Birth
  • Phone Number
  • Insurance and Identification Number
  • The name of your Primary Care Doctor (if you don't have one, name the last one you saw)
  • Name & DOB of the Card Holder, if applicable
  • Diagnosis or area(s) where you're experiencing pain
  • Let us know if you've had a recent MRI, X-Ray, EMG, etc.
  • Let us know if you're transferring your care from another pain practice
  • We will do our best to fit you in as soon as possible. The wait time for a new patient appointment is generally only about two weeks, dependent upon the doctor's schedule, as well as your own flexability. We also have convenient Saturday hours for those who can't make it during the week.
