If you have scheduled, or plan to schedule, your first visit to Ocean State Pain Management, feel free to download the new patient paperwork that is given at the time of your first visit. It's usually quite helpful, especially for patients with an extensive medical history or medication list, to fill out the paperwork in advance.  If you don't have access to a printer, feel free to stop by our office to pick it up.  (We do not mail out paperwork.)

Please note that the download is a Microsoft Word Document, so you will need Microsoft Word in order to open the file.  The document is compatible with most versions 1997-Present.
